Health Care Spending Account
A Health Care Spending Account offers you reimbursement for certain health care expenses before Federal, Social Security, and in most areas, state and local income taxes are withheld. A Health Care Spending Account can be used to pay for eligible health care expenses not covered under the medical, dental, and vision plans. You can also use the account to pay for health care expenses the IRS considers deductible on your Federal income tax return that cannot be reimbursed from any other means. This includes expenses for yourself and any person(s) you claim as a dependent on your tax return, regardless of if they are covered under a Verizon Wireless medical plan.

Your Spending Account™ MasterCard® Debit Card
Being reimbursed from your Health Care Spending Account is easy with the Your Spending Account™ MasterCard® debit card (YSA card).
Get the facts:

The YSA card is a prepaid debit card that works in conjunction with your Health Care Spending Account.
- With the YSA card, there’s no need to pay out of pocket for eligible health care expenses. The YSA card validates eligible expenses automatically for most transactions, which eliminates the need for paperwork. You can monitor all of your YSA transactions through the Your Spending Account™ website at www.vzwbenefits.com.
- Present your YSA card for eligible health care expenses at participating merchants and providers that accept YSA cards. Your expenses will be automatically validated and deducted from your Health Care Spending Account when you:
- Purchase eligible items from participating merchants and providers (see below for a list of eligible expenses).
- Use your YSA card to cover copays or other approved amounts.
- Make a repeating payment with your YSA card.
Note: The YSA card is for use with the Health Care Spending Account only – it does not work for the Dependent Care Spending Account.

Eligible Expenses
- Deductibles, copays, and coinsurance for medical and
dental services covered under your health care plans
- Amounts not covered by your medical plan because they
are in excess of Reasonable and Customary charges
- Charges above benefit maximums
- Childbirth classes
- Chiropractic services
- Contact lenses, vision exams
- Crutches
- Hearing aides
- Hearing exams
- Medical aids, such as orthopedic shoes
- Prescription eyeglasses, laser vision correction surgery
- Select over the counter (OTC) medications
- Smoking Cessation programs, prescriptions, and
over-the-counter therapies
- Transportation required to receive medical care (subject
to specific limits)
- Wheelchairs
- Any other health care expense you could deduct on your
federal income tax form, with the exception of contributions
for medical plan premiums

- Expenses for which you receive reimbursement through another
plan, such as the Verizon Wireless or another employer's
medical or dental plans
- Medicare or Medicaid
- Medical insurance premiums
- Non-prescription drugs or prescriptions related to cosmetic
procedures
- Cosmetic surgery (unless necessary to eliminate a deformity
related to a birth defect, a disfiguring disease, or a personal
injury as a result or an accident or trauma)
- Hair transplants
- Electrolysis
- Health club dues
- Teeth whitening
- Illegal operations or procedures
- Items paid or payable by insurance
- Marriage counseling
- Maternity clothes
- Nursing care for normal, healthy baby
